How CBD Affects Inflammation
CBD's anti-inflammatory effects work through several mechanisms:
TRPV1 channel modulation: CBD acts as an agonist at TRPV1 (transient receptor potential vanilloid 1) channels - receptors involved in pain and temperature perception. Desensitisation of TRPV1 reduces inflammatory pain signals.
TNF-ฮฑ and IL-6 suppression: CBD has been shown in cell culture and animal studies to reduce production of key pro-inflammatory cytokines including TNF-ฮฑ and IL-6. These are the same cytokines elevated in systemic inflammation, training-induced muscle damage, and conditions like rheumatoid arthritis.
2-AG/AEA (endocannabinoid) regulation: CBD inhibits FAAH - the enzyme that breaks down anandamide, an endocannabinoid with anti-inflammatory properties. Higher anandamide levels reduce inflammatory signalling.
NF-ฮบB pathway: CBD reduces activation of NF-ฮบB - the master regulator of inflammatory gene expression. This is a broad-spectrum anti-inflammatory mechanism relevant to multiple types of inflammation.
Human Evidence
The human trial evidence for CBD specifically in joint pain and inflammation is still early but directionally positive.
Arthritis: A 2019 pilot trial by Hammell et al. found topical CBD gel significantly reduced joint swelling, pain, and limb posture scores in an arthritis model. Human trials in rheumatoid arthritis are ongoing but preliminary data is positive for symptom reduction.
Exercise-induced inflammation: A 2021 RCT by Isenmann et al. examined CBD (150mg) taken for two weeks by recreational athletes. Results showed significantly reduced exercise-induced inflammatory markers (IL-6, creatine kinase) in the CBD group compared to placebo, along with faster perceived recovery.
Chronic pain with inflammatory component: Multiple trials in chronic pain populations (relevant to many men over 40 with tendinopathy, osteoarthritis, or back pain) show consistent reductions in pain scores and inflammatory markers. Topical vs Oral CBD for Joint Pain
For localised joint or muscle pain, topical CBD (cream or balm applied directly) has advantages over oral:
- Local concentration: Topical application delivers CBD directly to the affected area at higher concentrations than would reach from oral dosing at equivalent doses
- No systemic effects: Topical CBD produces minimal systemic absorption - useful for men who want local anti-inflammatory effects without any cognitive or sedative effects
- Onset: Faster onset for localised pain than oral CBD (15โ30 minutes vs 45โ90 minutes for capsules)
Oral CBD provides systemic anti-inflammatory effects - relevant for widespread inflammation, systemic recovery from training, or when targeting cortisol and sleep alongside the joint pain.
The most comprehensive approach: Oral CBD daily (25โ50mg) for systemic effects + topical CBD cream applied to specific joints or muscle groups post-training for localised benefit.
Dose for Inflammation in Men Over 40
- Systemic anti-inflammatory (oral): 25โ75mg daily, consistently. Anti-inflammatory effects are cumulative - a single dose won't produce meaningful reduction; 2โ4 weeks of consistent daily use is the appropriate evaluation window.
- Acute post-training recovery: 50mg post-session, taken with food
- Topical for localised pain: Apply directly to affected area 2โ3 times daily as needed
